Many teams on the National ACT COVID-19 Call have talked about new criteria they are explicitly asking clients about, and then presenting on in team meeting. These include: - COVID-19 symptoms, date of last assessment of symptoms, risk of infection or for complications from COVID-10
- Available ways of contacting the client (personal phone, via natural supports, shelter staff, etc.)
- Clients' access to basics (food, toilet paper, etc.)
- A shift in focus from bigger goals to problem solving basic needs/case management issues related to COVID-19

We have started an "Illness Tracker". We update with reported symptoms, last time contacted and for what, updated phone numbers as they arise and when and what active needs are outside of COVID symptoms. It has been very helpful and also makes sure we are reaching every client during this time. If they are reporting symptoms, the nurse (me) calls daily until symptoms resolve.
